German document, Ukrainian and Polish required all Polish residents to declare and sign on their non-Jewish status, according to the Natzi’s Classification, of laws and regulations that were announced by the Nazi Party in General government [für die besetzten Polnischen Gebiete] dated 24.7.1940. The document specifies in detail the definition of the term "Jew", including chapters of affiliation of the Jewish religious community and the affiliation of the grandparents of the Jewish race. The document discusses the status of the spouses of Jews, and the status of "Mischling" children born to mixed marriages of one spouse Jewish and one not. 

In the second part of the document there is a threat of punishment if affidavit turns out to be false.
